Gateway Health SM receives national recognition at Case in Point Awards

PITTSBURGH – For the second consecutive year, Gateway HealthSM has earned a prestigious national award at the Dorland Health’s Fifth Annual Case in Point Platinum Awards. Gateway Health was named winner in the Behavioral Health category for its successful Behavioral Health program and received an honorable mention in the Disease Management/Population Health category for its successful Gateway to Lifestyle Management Program.
The Case in Point Platinum Awards Program is a unique program that recognizes the most successful and innovative care management programs that successfully deliver programs that improve healthcare across the continuum of care coordination. The awards were presented this week at the Gaylord National Resort & Convention Center in Washington, D.C.  The following is a brief synopsis of the two care management categories for which Gateway was honored:
Winner – Behavioral Health in Managed Care
The Gateway Prospective Behavioral Health Management Program was recognized for improving behavioral health by assisting members with care transitions, treatment adherence, self-management and goal planning.  The model utilized takes a holistic approach to case management and is driven by an assessment encompassing six domains of member wellbeing which include: Behavioral, Economic, Environmental, Medical, Social, and Spiritual considerations.   The assessment is defined as the BEEMSS℠ tool, an acronym for each member focused domain.   The Behavioral Health Management team consisted of a psychiatrist, case managers and utilization managers dedicated to behavioral health.

Honorable Mention – Disease Management/Population Health in Managed CareThe Gateway to Lifestyle Management Team was recognized for effectively managing populations with chronic medical conditions including asthma, cardiac disease, chronic obstructive lung disease (COPD) and diabetes.
